A POLICE REPORT WAS FILED AND THE VEHICLE WAS DESTROYED. THERE WAS ONE INJURY AND TWO FATALITIES AS A RESULT OF THE CRASH. THE MANUFACTURER WAS NOT MADE AWARE OF THE FAILURE.
